Payment: We accept self pay as well as the insurances listed below

All major credit/debit cards accepted

Fees and co-pays must be paid prior to service


A $30 fee will be charged for all paperwork requests

This includes but is not limited to, FMLA paperwork, ESA letters, disability paperwork, attorney letters, etc.

Cancellation Policy

If you would like to cancel your appointment, it must be done at least 24 hours in advance or a $75 cancellation fee will be charged. This fee also applies for a "No Show" on the day of the appointment.
